Arriving somewhere new is a lot — forms, names, and finding the door. We take that hassle out of the first visit so you can spend the hour on the exam.

Pure Wellness offers chiropractic, acupuncture, and massage. First visits run about 60–90 minutes: history, posture, range of motion, exam, and — when you are a chiropractic case — an adjustment the same day if you want one. Bring photo ID and insurance cards.

Fill the online forms for your clinic before you arrive when you can. That is faster than a clipboard in the waiting room. Print the PDFs if you would rather bring paper, or call the desk if you want to fax a packet. Children under 18 need parental consent. New acupuncture patients complete the acupuncture questionnaire and consent even if they already see us for chiropractic.

Insurance, auto, and workers’ compensation.

We believe wellness care should fit a family budget. The office participates in most insurance programs and accepts auto-accident and workers’ compensation cases. If you have little or no chiropractic coverage, flexible payment programs can be arranged.

Most plans are accepted at every office, including auto, workers’ compensation, personal injury, Medicare, and Medicaid. Coverage varies widely — please verify insurance or call us. Staff verify and explain benefits before treatment, at no charge.

No chiropractic benefits?

ChiroHealthUSA discount membership.

If you do not have insurance or chiropractic benefits, there is still a way for you to receive the care you need. The ChiroHealthUSA program provides discounts on many of your chiropractic expenses. The program may be used by you and members of your immediate family. While this program is not insurance, it may be used to cover expenses not covered by a major medical health insurance policy.

Although welcome for treatment, these patients are excluded from offers: 1) MEDICARE, MEDICAID, TRICARE, and other government healthcare program participants and 2) personal injury and worker's compensation claimants. The exam charge includes a detailed verbal report of your exam findings delivered at a follow-up appointment. If you choose to utilize health insurance, the amount you pay may be lower than the advertised fees.

How long is the first visit?

Plan 60–90 minutes. We take a history, check posture and range of motion, examine, and — when you are a chiropractic case — adjust the same day if you want one. You get a detailed verbal report of findings at a follow-up visit.

What should I bring?

Photo ID, insurance cards, and completed forms. Children under 18 need a parent or guardian. If this is a car crash or work injury, say so when you book.

Can I fill forms out at home?
