Although welcome for treatment, these patients are excluded from offers: 1) MEDICARE, MEDICAID, TRICARE, and other government healthcare program participants and 2) personal injury and worker's compensation claimants. The exam charge includes a detailed verbal report of your exam findings delivered at a follow-up appointment. If you choose to utilize health insurance, the amount you pay may be lower than the advertised fees.

What happens after you submit

Your request lands with the clinic you selected and a member of that front desk calls you back to confirm a real time slot. Preferred day and time are treated as a target, not a booking — clinics run split days with a midday break, so the exact hour you asked for may not exist that week.

On the call they will confirm what is wrong, check whether you need a longer new patient block, and take your insurance details if you have them. Benefits are verified before your first visit at no charge, so you know what you are paying before you sit on the table rather than after.

If nothing suitable is available at your first-choice office, they will offer the nearest clinic with an opening. Our five offices share intake forms and pricing, so switching between them costs you nothing.

Is submitting this form a confirmed appointment?

No — it is a request. The preferred day and time you choose tell the desk what you are aiming for, and someone from your chosen clinic calls back to confirm an actual slot. If you need to be seen today, call instead.

How soon will someone get back to me?

During clinic hours, usually the same day. Requests submitted overnight or at the weekend are picked up when that office next opens, so check the clinic's hours if you are working to a deadline.

How long is the first visit?

Between 60 and 90 minutes. You will go through your history, a posture screening, measured range of motion, and a hands-on exam. If you are a chiropractic case and you want an adjustment the same day, that happens too. A full verbal report of findings comes at your follow-up.

What will it cost?

The new patient exam is advertised starting at $78 and a consultation on its own is $39. If you use insurance, staff verify your benefits at no charge before care starts and the amount you pay may be lower than the advertised fee.

What should I bring?

Photo ID, your insurance cards, and any recent imaging or reports. Printing the intake forms and completing them at home saves you working through a clipboard while you are in pain. Patients under 18 need a parent or guardian to consent.

Can I ask for a specific doctor?

Yes. Put the name in the message field and the desk will book you into that doctor's schedule. If you are not sure who to ask for, describe the problem instead and they will match you.

Where does this form actually go?

To the front desk of the clinic you select, through our internal system. It is not a shared public inbox and it is not a call centre. Please do not include insurance member IDs or detailed medical history in the message field.
